To help students to develop the ability to use critical reflection to examine their service learning experience and to act on what they have learned. Follow the template below, and include headers labeled for each part 1-3. 
Title page according to APA format
Part 1.
Describe the service learning activity including the place or location, dates, times, and purpose of the activity. Who participated?  What was your role? Who are the benefactors, and how did they benefit from your service and the activity?
Describe three ethical principles discussed in the Belmont Report.
Part 2
Examine your service learning experience and include in your discussion the following:
How did you grow personally from the experience in terms of your strengths and weaknesses?
What goal did you set for yourself and was it achieved?
Which principles of civic responsibility relate to your experience, and why?
Interpret the relationship between the Belmont Principles and the federal regulations.
Illustrate how the Belmont Principles and federal regulations correlate to your service-learning experience(s).
Part 3
Articulate what you learned through the service learning experience(s) and academic learning and address the following:
What did you learn?
How did you learn it?
Why does it matter?

Review the Rubric used for grading of this assignment
Headers for each Part-1-3 are required
Each Part 1-3, should be at least one page in length, double spaced, Times New Roman or Arial font only, no larger than 12
